Terms Of Service

By booking or attending an appointment at Embody Osteopathy, you agree to the following terms. These terms apply to all patients of this clinic.

 

Scope of Services

 We provide osteopathic care in the form of: a thorough history taking, physical assessment, diagnosis, osteopathic treatment and management advice in line with the standards of the Osteopathy Board of Australia. Our treatment services may include: soft-tissue massage, joint mobilization, spinal manipulation, muscle energy techniques, counter-strain, functional techniques and dry needling. We may also provide management advice and education relevant to your diagnosis, this may include exercise rehabilitation, ergonomic advice or general exercise advice. Our care does not replace emergency medical attention. If you have a medical emergency, call 000 or attend your nearest hospital.

Appointments, Bookings and Forms

Appointments can be made online, by phone or in person. It is required of patients to arrive on time, or not later than 10 minutes after their appointment time. If you arrive late, your appointment may be shortened by that duration of time but the full appointment fee will still apply.

Digital online forms are sent to you shortly after booking an initial appointment, it is your responsibility to complete this form promptly and with accuracy. This form is saved into your medical file on our practice management system ‘Cliniko’. For information regarding your privacy, please refer to our Privacy Policy.

We reserve the right to refuse or discontinue treatment if it is unsafe or inappropriate for you. Circumstances might include: presenting of red flag symptoms or absolute contraindications to treatment, need for further investigations or a lack of diagnosis after assessment.

 

Fees and Payment

All fees are payable on the day of service unless otherwise arranged. We accept payment via Eftpos and Credit card. If claiming Private Health Insurance or Medicare Rebate (via Enhanced Primary Care Plan), we can process your claim on site via Hicaps machine. We require a physical card or digital card of your private health insurance/Medicare card. Alternatively, you will have to complete your claiming process via the health insurance/Medicare App or website after your appointment in your own time. Any remaining gap balance is payable by you on the day of claim.

 

Cancellation and No-Show Policy

We require 24 hour’s notice if you need to cancel or reschedule your appointment. Cancellations or no-shows with less than 24-hours notice may incur a fee of the appointment price. We understand emergencies and inconveniences arise, and we will approach this with compassion. Late cancellations/no-show fees will apply for patients who repeatedly offend.

 

Patient Responsibilities

It is the patient’s responsibility to complete new patient form with accurate and up-to-date personal information, including name, contact information, Medicare details and health care information.

Patient’s must provide an accurate and complete medical history, including previous diagnosis of health conditions, medications and management of said health conditions.

It is the patient’s responsibility to:

• Inform us promptly if health status changes.

• Follow reasonable management advice and safety instructions.

• As questions if aspects of your care are not understood.

• Understand their rights as a patient of the clinic.

 

Informed Consent 

Before your treatment begins, we will provide:

• A diagnosis, explanation of diagnosis and proposed management plan

• Explanation for osteopathic treatment techniques for provisional diagnosis

• Possible risks, benefits and alternative treatments

It is your right to ask questions regarding the treatment throughout your entire consultations, and it is your right to withdraw or refuse treatment at any stage during your consultation. We will not continue treatment without your initial consent, and we will continue to ask for your consent at different stages throughout the treatment.

 

Health and Safety

Please reschedule your appointment if you are unwell with contagious symptoms (fever, active cough, vomiting, infectious rash). We may refuse treatment if you present with illness or are under the influence of drugs or alcohol.

 

Confidentiality and Privacy

Your personal information is handled in accordance with the Privacy Act 1988 and Australian Privacy Principles. We will not share your information without your consent unless required by law or necessary for your healthcare (e.g referral to another provider)

Limitation of Liability

While we take all reasonable care, we are not liable for any loss, damage, or injury arising from:

• Your failure to provide accurate health information.

• Your failure to follow management advice.

• Outcomes outside our reasonable control.

 

Complaints and Feedback

Changes to These Terms

We may update these terms. The latest version will always be available on our website.

 

Governing Law

These terms are governed by the laws of Victoria, Australia.
