About this Policy

Embody Osteopathy ABN: 38240344551 trading as Embody Osteopathy (“us”, “we”, or “our”) recognizes the importance of your privacy and respects your right to control how your personal information is collected and used. We are committed to protecting your privacy in accordance with the Privacy Act 1988 (Cth) (the Privacy Act) and with the Australian Privacy Principles as set out in the Privacy Act and described the way that we may collect, hold and disclose personal and health information.

In this policy Personal Information means any information that may identify you, or by which your identity might be reasonably determined. The information you provide us may include, amongst other things, your name, address, email address and phone number.

Sensitive Information means any information about an individual’s racial or ethnic origin, political opinions, memberships of a political organization, religious belief or affliction, philosophical belief, membership of a professional or trade association, membership of a trade union, sexual preference or practices, criminal record or health record.

Information Collection

Your personal information is collected when your book an appointment at Embody Osteopathy (online, via website, SMS, or email). Further information is collected when you complete a patient intake form prior to initial consultation, atten a consultation or communicate with us, or are referred by another healthcare provider I.E general practitioner. The following information is collected when you book in with us:

● Full name, address, phone number, email address, emergency contact details

● Date of birth and Medicare/private health fund details

● Medical history, treatment notes, referrals, medical imaging reports

● Payment details (for processing payments), including credit card/Eftpos details.

Use and Disclosure

Personal information collected by us will generally only be used and disclosed for the purpose it was collected. To maintain updated account and contact details, provide safe and effective osteopathic care, communicate with you about upcoming appointments and treatment plans, process payments and manage clinic operations, and comply with legal, regulatory and professional obligations.

We may use and share non-personally identifying information about clients for market analysis, research, marketing or other purposes.

We will not disclose, sell, share or trade your personal information to any third partied unless we first receive your consent.

We may provide health information to other medical service providers, such as your general practitioner or specialist medical practitioners. We will only supply this information with your consent, or circumstances where it is required for the delivery of health services, such as referral to another health practitioner (Enhanced Primary Care Plan). We may disclose information to Medicare for claims processing, or with regulatory or legal authorities where required by law.

Information Storage

Your personal and health information is securely stored using industry-standard systems, including our practice management software (Cliniko), which complies with the Australian data protection laws.

We take reasonable steps to protect your data from misuse, interference, loss, and unauthorized access.

Accessing and Correcting Your Information

You have the right to request access to your personal information and request corrections if it is inaccurate. Please contact us in writing, and we will respond within a reasonable timeframe.
